Treating Puffy Nipples
Puffy Nipples differs from Classic Gynecomastia in that the nipple and areola are much more cone shaped causing the areola to appear “Puffy”. Normally, the areola is even with the surrounding chest skin. With Puffy Nipples, the areola projects outward. This is especially true when the temperature is hot. Patients routinely complain that their nipples are too visible in thin clothing during hot weather.

Patients with Puffy Nipples are often thin and in very good shape. This is especially frustrating because no matter what they do they can not get rid of the conical shape.
What causes Puffy Nipples?

Puffy Nipple is usually caused by an excess amount of breast tissue directly underneath the nipple/areola. This breast tissue pushes the nipple outward making it overly visible in thin shirts. In addition, it pushes the entire areola outward so that it is no longer even with the surrounding skin. This is what causes the “puffy” appearance. Once again, this is particular evident in warm weather. What distinguishes Puffy Nipple from Classic Gynecomastia is that Puffy Nipple does not have nearly as much fat. Liposuction alone is less likely to be effective in puffy nipples than with other types of gynecomastia. Liposuction alone will reduce the overall volume, but it may, in fact, make the chest more cone-shaped.

Notice how the chest in Puffy Nipple often does not round out as much as classic gynecomastia. The chest fold maintains a relatively horizontal orientation. Rounding of the chest is from excess fat or breast tissue along the bottom and lateral part of the chest.
On the profile view, notice how the nipple and areola projects outward from the skin giving a “puffy” appearance. The excess breast tissue directly beneath the nipple also causes the nipples to often be pointy. This makes the nipples more noticeable especially in t-shirts.

The cut out view clearly shows why the Puffy Nipple is conical in shape. The breast issue is located directly below the nipple/areola. This creates a cone with the nipple on top. Also apparent is the relative lack of fat when compared to classic gynecomastia. This is why the chest is not so rounded out. Once again, removing the fat with liposuction only and not removing the breast tissue may make the nipple more cone shaped.
