Treating Enlarged Male Breasts - Classic Type
Classic Gynecomastia is the most common presentation. The enlarged breast is actually a combination of excess breast tissue and an excess of fatty tissue.

Notice how the enlarged chest rounds out at the bottom compared to the more horizontal orientation of the ideal chest. The reason is that the excess breast tissue prevents the underlying pectoralis major muscle (which has a horizontal orientation) from showing through. Instead, the excess tissue hangs downward in a more round appearance.

The most revealing view is the oblique as shown above. It shows the increased projection of the chest especially at and below the nipple. Often, the distance from the bottom of the areola to the chest fold is increased. In young men, this usually snaps back, but in older men it may not.

Notice that the nipple and areola, despite being overly projected, are flush with the surrounding skin. This is what distinguishes Classic Gynecomastia from Puffy Nipple.

From an anatomic standpoint as shown above, you can see how Classic Gynecomastia is a combination of excess breast tissue (white) and excess fat (yellow). Typically, the excess breast tissue is located directly under the nipple. Those affected can often easily feel it by pinching the glandular tissue underneath the nipple. It feels firm and rubbery; much firmer and more gritty than fat.
Keep in mind, EVERY man has some breast tissue located under the nipple. This is normal. Having breast tissue is not a reason to have surgery. Only if it bothers you should surgery be considered.
The excess fat is primarily underneath the breast tissue but is also often dispersed within it as well. It is usually spread out over a larger area.
